Chronic Disease Management

Service Description

We provide chronic disease management for the following conditions as part of the HSE scheme currently being rolled out for patients over 70, but soon to be extended to all age groups: - Type 2 Diabetes Mellitus - Ischaemic Heart Disease - Asthma/COPD. This involves 2 visits per year to the GP/Practice Nurse and an individualised care plan is drawn up for each patient, taking into account their specific needs, following assessment.
